What to expect in the first visit.
We ask you to arrive 15 minutes before your appointment. You can download the patient information form and the HIPPA policy form by clicking on these links. Your first visit to our clinic involves:

1. History and Physical. Dr Afzal will obtain a detailed history of your symptoms, he will review your medical condition and perform a physical exam.

2. Ultrasound. Based on your symptoms and presentation, a comprehensive ultrasound exam of your leg veins will be performed. Ultrasound helps look at veins not visible to the naked eye. Most patients are surprised at the extent of their vein disease. Ultrasound exam helps identify the path and the direction of the blood flow and helps in formulation of a comprehensive treatment plan.

3. Treatment Plan. After the complete and comprehensive evaluation Dr Afzal will discuss with you the treatment options and the expected results.

4. Waiting for insurance approval. At the end of the day, we will write a letter to your insurance company for approval of your treatment. It usually takes 3 weeks for the insurance company to respond to your prior authorization requests.
